Transition House Release Meaning

FDC’s 2026 Polk Correctional Institution audit identifies The Transition House of Bartow as a contract facility within Polk CI’s administrative and daily-population grouping. It is a state-system community-confinement or transition placement. It is not a PCSO jail, and it has no separate public released-inmate roster documented in the research.

That status makes the word “release” especially easy to misread. Transfer from an FDC prison to the Bartow house may be a step toward community return while the person remains within a correctional program. Departure from the house may lead to another FDC site, community supervision, or final discharge. The named person’s FDC status and release history, not the presence or absence of a PCSO booking, define the transition.

Search Transition House Releases

Start at the FDC Corrections Offender Network. The menu separates current inmate, inmate release, supervised population, and absconder records. Search the current inmate database to identify a present state location. Search Inmate Release Information when the person is believed to have completed an FDC incarceration period.

1. Use the six-character DC number when available; otherwise search first and last name with alias matching.
2. Compare name, birth date, sex, race, photograph, and DC number before selecting a record.
3. Read the current location and incarceration history to see whether Bartow was a placement within continuing FDC custody.
4. Look for a past Release Date and matching Date Out-Custody in the FDC release database.
5. Check Supervised Population for a current supervision type, office, start date, or termination date.

A person can be outside a prison gate and still under state supervision. Parole, conditional release, probation, and community control are distinct legal statuses. The FDC result should be read as a sequence, not reduced to one location field.

Note: A new FDC location is evidence of transfer, while a past release date and matching out-custody date document the state incarceration transition.

Transition House Status Records

The public FDC systems answer different parts of the question. The inmate detail identifies current or prior incarceration information. The release database retains a historical state release milestone. The supervised-population database can show continuing state control outside prison. FDC’s public release details do not always state a single narrative reason, so records or a Commission order may be needed for the exact mechanism.

FDC says release information updates nightly. Short timing differences can appear among current location, release, and supervision views. Save the date of the search and recheck before assuming a conflict. A historical release detail is not a live report of the person’s later whereabouts.

Transition House Records Contact

No unique public release-search contact for the Bartow house appears in the research. The records route therefore stays with FDC, which owns the state custody and supervision record. The Bureau of Admission and Release handles questions attached to the release database, and FDC’s public-records portal is the route for existing state documents.

Give the full name, aliases, date of birth, and DC number. Add the approximate Bartow placement and release dates, conviction county, and case number. Ask for existing placement history, dates in and out, release or discharge record, supervision status and termination, sentence computation, and any relevant public Commission order. Do not ask PCSO to certify this FDC placement.

Bartow House and Polk CI

The 2026 FDC audit is the authoritative source for the contract relationship. It says Polk CI contracts with The Transition House of Bartow and includes it in the institution’s daily population count. The same administrative grouping also includes the Polk Main Unit, Polk Work Camp, and sites outside Polk County.

Administrative grouping should not be mistaken for one physical campus. The Bartow house is local to Polk County, while the person’s state record should show the applicable placement and later movement. When a record changes from Polk CI to the contract house, that may reflect a transition within the FDC framework rather than a final discharge.

Verify a Bartow House Discharge

Confirmation should join three facts: the correct identity, the recorded end of the FDC incarceration period, and any supervision that follows. A past Release Date alone answers when the listed state custody period changed. It does not necessarily show that parole, conditional release, or another supervision term ended.

Use the supervised-population database after the release search. If it shows an active supervision type and office, the person is outside prison under stated FDC supervision. If it shows a termination date, that date is relevant to the end of supervision. The Florida Commission on Offender Review may hold the order when parole or another Commission-controlled form of release applies. Bartow House Historical Records

FDC’s OBIS public data provides monthly release cohorts beginning October 1, 1997. Those files can contain aliases, state offenses, detainers, basic release information, and incarceration history. Facility Description and Prison Release Date may help trace an older state event involving Polk’s FDC network.

The archive is a fallback, not a Transition House release feed. A person must still be matched by DC number and identifiers, and the record must be interpreted against the correct incarceration period. Confidential material is excluded, while sealed or expunged records are removed. If the desired placement detail is not public online, ask FDC for the existing underlying record.

Note: Monthly archive timing differs from the nightly release search, so record the source and update date when comparing results.

Transition House ICE Boundaries

Research on Polk County includes a 2019 ICE and PCSO warrant-service memorandum and a broader 2025 PCSO task-force memorandum. Those are sheriff agreements about county activity. They do not show that The Transition House of Bartow, its private operator, or FDC follows the same transfer procedure. Applying them to this state contract placement would blur agency boundaries.

If an FDC record indicates another-agency custody or a detainer, request the relevant state record and verify the named receiving agency directly. An ICE locator result may answer current or recent immigration custody, but it does not replace the FDC release history. Conversely, a missing ICE result does not prove a person who left this Bartow placement entered the community without continuing state or federal obligations.

Transition House Release Definitions

Several terms can appear around this placement and should remain distinct. Plain definitions keep a custody change from being overstated.

Community confinement

A structured placement outside a conventional prison that can remain part of correctional control.

Transfer

A move between facilities or custodians without necessarily ending custody.

Release from incarceration

The recorded end of a prison or custody period, sometimes followed by supervision.

Supervision

Legal oversight in the community under stated rules and an assigned agency.

Discharge

The recorded end of the correctional status at issue, which may occur after prison release.

These distinctions are why the county sheriff’s current roster cannot answer the Bartow house question. The person’s DC number, FDC dates, location history, and supervision status form the correct state record chain.

Match the Bartow FDC Record

Use the DC number whenever possible because the state identifier follows the person across facilities and supervision records. When only a name is known, retain alias matching and compare the photograph, date of birth, sex, race, conviction county, case number, sentence date, and offense. A location label without a firm identity match is weak evidence.

Review each incarceration period separately. The date out of one placement may precede a move within the same state framework. The release history must show the relevant state milestone, and the supervision search must be checked for continuing control. If the public detail lacks enough information, give FDC the identifiers and ask for the existing placement and release documents.
